🏒 Hockey
- USA Hockey Registration (annual, required for players and coaches)
- SafeSport Training (required for players 17+, coaches, managers, and volunteers)
- Medical Release / Waiver (often league-specific)
- Birth Certificate or Proof of Age
- Player/Parent Contract or Code of Conduct
🏈 Football (Youth Tackle / Flag)
- League Registration (Pop Warner, AYF, or local association)
- Physical Examination / Medical Clearance
- Concussion Awareness Form (state law in many areas)
