Managing paperwork can feel like a second job when you’re running a youth sports team. From medical forms to proof of age to league-specific requirements, missing even one document can sideline a player until it’s resolved. 🏒 Hockey

  • USA Hockey Registration (annual, required for players and coaches)
  • SafeSport Training (required for players 17+, coaches, managers, and volunteers)
  • Medical Release / Waiver (often league-specific)
  • Birth Certificate or Proof of Age
  • Player/Parent Contract or Code of Conduct

🏈 Football (Youth Tackle / Flag)

  • League Registration (Pop Warner, AYF, or local association)
  • Physical Examination / Medical Clearance
  • Concussion Awareness Form (state law in many areas)
  • Birth Certificate / Proof of Age
  • Parental Consent & Waiver of Liability
  • Equipment Agreement / Uniform Deposit (some leagues)

  • US Youth Soccer / US Club Soccer Registration
  • Medical Release / Waiver
  • Concussion Information Sheet (required in many states)
  • Birth Certificate / Passport for ID Cards
  • Code of Conduct Agreement

🥍 Lacrosse

  • US Lacrosse Membership (player and coach)
  • Medical Release / Waiver
